MENTAL HEALTH PROGRAM PATH TEAM

The Mental Health Program PATH Team (Projects for Assistance for Transition from Homelessness) does outreach for individuals who are homeless, at imminent risk of becoming homeless, have a serious mental illness, or have co-occurring mental illness and substance abuse.  Once the PATH team reaches a client, the Gateway mental health program allows the client to stabilize, create an individual action plan, establish a medication compliance regimen, and participate in groups and classes that lead to independent living and self-sufficiency.

  • Maximum 3-month program
  • Housed on the 2nd floor of the Gateway Center
  • Program has 42 beds for men who have been diagnosed with mental health issues
  • Clients receive 2 nutritional meals if they are on-site during meal serving time
  • Gateway Center Mental Health Specialists provide therapeutic counseling to program participants
  • Program assists client into appropriate housing – supportive or permanent
  • Mental Health Specialists are regularly available at the CEC or by referral from the Services Desk or residential programs at any time
